Contract / Travel Physical Therapist (Up to $2,200 / wk)

Location : Wisconsin

Setting : Mixed - Inpatient Acute, Swing Bed, Outpatient, Skilled Nursing Facility (SNF) (Weighted more heavily towards outpatient;

Current staff willing to adjust load as needed / desired)

Start Date : ASAP

Schedule : Monday to Friday, 40 hours guaranteed. Flexible hours, mainly 8 : 00 AM - 4 : 30 PM, but will accommodate adjustments.

Case Load : 8 patients per day

Culture : Small facility in a rural setting, fostering a supportive and collaborative environment. No strict productivity standards.

Physical Therapists work closely with in-house providers.

Job Description :

Our facility in Wisconsin is seeking a dedicated and experienced Contract / Travel Physical Therapist to join our team. As a Physical Therapist in our mixed setting, you will have the opportunity to work in various environments including inpatient acute care, swing bed, outpatient, and skilled nursing facility (SNF), with a focus on outpatient care.

We prioritize patient care and believe in giving our therapists the flexibility to provide individualized treatment plans without strict productivity constraints.

Responsibilities :

• Evaluate, plan, and administer comprehensive physical therapy modalities / regimens to patients in a variety of settings.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Collaborate closely with in-house providers to ensure coordinated and effective patient care.
• Maintain accurate and up-to-date patient records.

Requirements :

• Valid Physical Therapist license in the state of Wisconsin.
• Experience in a mixed setting, particularly outpatient care, is preferred.
• Strong clinical and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Excellent communication and documentation abilities.
