Position Summary
Congreso’s Prevention Department seeks Case Managers with direct-service experience and a passion for working with children, youth, and parents.Prevention Case Managerscollaborate with families to identity needs and achieve goalsrelated to home safety, child well-being, improving school attendance, parenting skills, and overall self-sufficiency.This work involves conducting a thorough assessment of the strengths and needs of the families,developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs or services. Prevention Case Managers guide families through the process of enrolling in supportive programs using a strengths-based approach and respecting the family’s self-determination.This work also involves supporting families through advocacy and providing essential resources.
Prevention Case Managers will specialize in serving families from one of two programs:
Truancy:Case Managers are assigned to specific school sites and serve children with low school attendance who are at-risk for Truancy Regional Court. This involves collaborating with school personnel, frequent monitoring for monthly school and home visits, advocating for students in court, and working with families to eliminate barriers to academic success.
Family Empowerment Center: Case Managers enhance the protective factors of families at-risk of child welfare involvement. This involves either monthly or twice-weekly home visits depending on tier, collaborating with a DHS social worker if there is an active case, and connecting families to resources that help improve overall well-being.
This position reports to a Program Coordinator or Manager.
The Prevention Case Managerapplies the principles of the organization’s signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee-focused experience that is appreciative of the unique strengths, values and needs that each employee brings to the workplace; and by using data to inform and drive decision making. Attention is given to compliance, quality assurance, and best practices as they relate to employee experience, records management, and data integrity and usability.
Essential Functions
CASE MANAGEMENT
Provide in-home services to at risk families or individuals as determined by referral. This would include a hybrid caseload ranging from low to high risk referrals.
Provide case management services – conduct home visits, complete family assessments,create service plan, manage crisis intervention, advocate, connect to resources, and keep case files according to program standards.
Coordinate with school personnel, DHS social workers, and service providers as needed to communicate progress and work toward successfully connecting families to resources.
If serving Truancy clients, attend court as advocates and complete court reports timely.
Safeguard the confidentiality of client families.
Promptly report all conflicts among client families, or serious incidents involving client and families within 24 hours of the incident.
Provide services and support outside of the normal working hours to accommodate the family’s needs when necessary. Be available on-call 24/7 for client emergencies (occasional).
Regularly attend and actively participate in staff meetings, supervision, staff training sessions, case reviews, etc.
COMPLIANCE & QUALITY ASSURANCE
Consistently keep updated, detailed client records in databases as required by program standards.
Keep quality case files that pass all internal and funder file audits.
Complete assessments, home visits, and other required activities within the standard timeframe.
Ensure that program quality assurance tools are used and updated according to deadlines.
Perform related duties as assigned.
Non-Essential Functions
Attend relevant workshops or join professional groups as necessary to maintain professional knowledge.
Adheres to the Congreso’s security guidelines and ensures the appropriate handling of sensitive information.
Facilitates and attends relevant staff meetings to promote communication and execution of goals.
Completes special projects specific to the function of the department or as needed for the department as directed by Supervisor.
Other duties as assigned within the scope of position expectations.
Minimum Experience, Education, and Qualifications
Minimum Education:Bachelor’s degree in Social Work or related field, or Master’s degree in Social Work.
Minimum Experience:
1-2 yrs. Case Management experience in providing supportive services in a community setting.
Excellent oral and written communications skills; Bilingual (English/Spanish) preferred.
Experience working with TANF recipients preferred.
Familiar with the North Philadelphia/Latino community needs and knowledge of resources within the community.
Understanding of legal criteria for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements.
Ability to provide prevention services with a high level of ethical standards of conduct, cultural sensitivity and within appropriate boundaries and limits.
Possesses strong interpersonal skills.
Ability to operate a computer and use a variety of common software programs including Microsoft Office, spreadsheets, ETO software (agency database), and customized databases.
Strong written and verbal communication skills and effectively communicate with individuals and groups.
