TheFEC Diversion CM/Social Worker willcollaborate with families to identify needs and achieve goals related to home safety, child well-being, improving school attendance, parenting skills, and overall self-sufficiency. This work involves conducting a thorough assessment of the strengths and needs of the families, developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs or services. FEC Diversion CM/Social Worker guide families through the process of enrolling in supportive programs using a strengths-based approach and respecting the family’s self-determination. This work also involves supporting families through advocacy and providing essential resources.FEC Diversion CM/Social enhances the protective factors of families at-risk of child welfare involvement. This involves either monthly or twice-weekly home visits depending on tier, collaborating with a DHS social worker if there is an active case, and connecting families to resources that help improve overall well-being.

This position reports to the FEC Program Manager. This is a HYBRID position.

Essential Functions

  • RepresentsCongreso’s mission, values, and commitment to diversity, equity, inclusion, and belonging and will be an ambassador of our work.

  • Provide in-home services to at-risk families or individuals as determined by referral. This would include a hybrid caseload ranging from low to high-risk referrals. 

  • Provide case management services – conduct home visits, complete family assessments,create service plans, manage crisis intervention, advocate, connect to resources, and keep case files according to program standards.

  • Coordinate with school personnel, DHS social workers, and service providers as needed to communicate progress and work toward successfully connecting families to resources.

  • Safeguard the confidentiality of client families.

  • Collaborate with FEC Coordinator to complete ASQ Assessments and any needed referrals based on ASQ results. 

  • Promptly report all conflicts among client families, or serious incidents involving clients and families within 24 hours of the incident.

  • Provide services and support outside of traditional working hours to accommodate the family’s needs when necessary. Be available on-call 24/7 for client emergencies (occasional).

  • Regularly attend and actively participate in staff meetings, supervision, staff training sessions, case reviews, etc.

  • Perform other duties as assigned.

Supervisory Responsibilities:

This position supervises support staff (TBD).

Minimum Experience, Education, and Qualifications

Minimum Education: 

Master’s degree in social workor LSW.

Minimum Experience:

1-3 years’ experiencerequired.

Familiarity with the child welfare system.

Trauma-focused trainingis preferred but not required

Certification/License:

A mandated Reporter training certificate is required and may be completed post-hire.
