Looking for a full-time Service Coordinator/Social Worker to rotate between two properties.

Qualifications:
- A bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable
- Demonstrate working knowledge of supportive services for senior citizens
- Strong writing and organizational skills
Job Duties:
- Serve four properties, providing referral services for residents in need of support
- Perform initial assessments concerning functioning capabilities, link them with service providers and monitor the provision of the service
- Establish links with agencies and services in the community
- Help residents build support networks with other residents, family and friends
$17/hour Medical, Dental, Vision, Life, 401k, and Mileage Reimbursement
