INTRODUCTION:

The Siletz Diabetes Program Department’s purpose is to improve wellness to prevent, manage, or reverse diabetes. The core of the physical activity component is based at the Tillicum fitness center. This position’s role is to staff and maintain Tillicum, lead other physical activity opportunities and serve on the diabetes team.

RESPONSIBILITIES:

• Staff, clean, maintains Tillicum exercise facility per scheduled assignments.
• Lead/teach/plan exercise opportunities for groups or individuals.
• Use motivational interviewing techniques to help clients set wellness goals. Develop follow-up plans.
• Keep records and data that support the diabetes program.
• Provide excellent customer service.
• Other duties as assigned.

Requirements

REQUIREMENTS:

High School diploma or GED.

Knowledge of:

• Healthy life style practices. 
• Knowledge of the unique health concerns of Native Americans.
• Diabetes, exercise physiology or ability to attain within 6-months of hire.

Ability to:

• Work a flexible schedule including evenings and/or weekends.
• Build and maintain client rapport and trust through a nonjudgmental attitude, advocacy, timely follow-up to client requests, and unconditional positive regard.
• Maintain personal and professional boundaries.
• Ability to exercise good judgment in situations significantly impacting client and/or own safety and well-being.
• Model positive health behaviors.

Other:

• Current First Aid /CPR or obtain within 15-days of hire. 
• Basic ACE online training Fitness Service Specialist (for gym safety) or must complete within 60-days of hire.
• Committed to motivating and supporting patients making lifestyle changes. 
• Motivational Interviewing training. 
• Training will be offered for American Council on Exercise (ACE) certification as either “Personal Trainer” or “Group Fitness Instructor” or Native American Fitness Council certifications as “Personal Trainer”. Incumbent will be expected to complete training courses.
