We are Currently Seeking PRN Physical Therapists, Physical Therapy Assistants along with Occupational Therapists and Certified Occupational Therapists

Qualifications:

• Licensed as a Physical Therapist in the Commonwealth of Pennsylvania
• Licensed as a Physical Therapist Assistant in the Commonwealth of Pennsylvania
• Licensed/registered as an Occupational Therapist in the Commonwealth of Pennsylvania
• Certified as an Occupational Therapy Assistant in the Commonwealth of Pennsylvania

Essential Duties and Responsibilities

• Development and modification of individualized, goal-directed treatment plans
• Provides therapeutic actions to relieve pain and/or develop or restore/regain maximum independence
• Implements treatment plans, monitors resident response to intervention, and modifies treatment to attain goals in accordance with physician orders, plan of care, and discharge plans
• Develops appropriate home or community programming to maintain and enhance the performance of the resident in their own environment
• Documents results of assessment, treatment, follow-up, discharge, and billing notes in an accurate, timely manner in accordance with Standards of Practice, state and federal regulations, and policies/procedures
• Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
