What is a Home Health Care Medical Social Worker?
A Home Health Medical Social Worker, or Home Health MSW, travels to a patient’s home to administer social work services to patients on an intermittent basis in their place of residence. This is performed by physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
What does a Home Health Medical Social Worker do?
Home Health Medical Social Workers provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. Some tasks include:
- Plans/coordinates all social services within the Agency
- Documents all patient/family services provided as required by Agency policy
- Assists physician and other team members in understanding the significant social and emotional factors related to health problems
- Participates in the development of the plan of care
- Prepares clinical and progress notes
- Works with the family
- Participates in discharge planning
What are the qualifications for a Home Health Medical Social Worker?
- A person who has a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
- One year of social work experience in a healthcare setting
- Strong assessment and care planning skills
Do you need home health care experience to be a Home Health Medical Social Worker?
One year of previous home health experience is required.
Do Home Health Medical Social Workers work alone?
Home Health Medical Social Workers are autonomous, but they are not alone. All field staff work with supportive clinical supervisors and are a vital part of the collaborative clinical team.
