CONCERTO is Hiring a Clinical Operations Dialysis Nurse Near Livonia, MI

POSITION SUMMARY:

This full-time exempt position requires ability to support local leadership team and oversees multiple sights with no direct reports.

Under the direction of the Vice President of Operations and Regional Director of Operations. The Clinical Operations Nurse collaborates with Vice President of Operations, Regional Director of Operations, Regional Manager, and all members of the multidisciplinary team to drive Concerto quality standards and foster a team approach to patient care. The Clinical Operations Nurse reports directly to Regional Director of Operations. Accountable for customer service, staffing, assisting operations, and ensuring the team is following CRS policies and procedures.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

Must be able to do light lifting of at least 20 lbs. Requires mental, visual, and auditory attention to patients, customers, and staff. Work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious disease/materials. Requires travel between facilities.

JOB RESPONSIBILITIES:

STAFFING & SUPPORT:

  • Assist with education and training of current and new concerto teammates
  • Provide continuing education and in-service opportunities as needs identified
  • Attend regular staff meetings to share information and concerns
  • Staffing floor in emergency circumstances (Possibility of third shift)

QUALITY:

  • Assist with oversight of facility outcomes and aids RM
  • Works with the Vice President, Regional Manager, and members of the Interdisciplinary Team (IDT) team to solve quality improvement challenges and implement action plans for improvement.
  • Provide follow up from clinical operations calls on completed medical chart audits and any additional audits
  • Assist with quality standards through utilization of KDOQI guidelines, regulatory requirements and CRS professional standards of care
  • Assist with assurance of quality patient outcomes utilizing CRS professional standards of care
  • Assist RM with periodic chart audit and data collection for analysis of quality indicators
  • Assist in tracking data to determine need for policy change and/or facility practice
  • Support with follow up on QAPI action plans
  • Assist with follow up of facility audits and provide support in developing a corrective action plan with RM
  • Provides support to the Vice President and Regional Manager to ensure implementation of processes and resolution of identified deficiencies.
  • Participates in Quality Assurance and Performance Improvement (QAPI) meetings, Comprehensive Interdisciplinary Patient Assessment (CIPA)/Plan of Care (POC) and Nursing Home QA/QI meetings as requested

REGULATORY & SAFETY:

  • Support Follows up on Adverse Occurrence reports and assist RM with education staff on properly completing AOR
  • Reports and follows up on equipment repairs
  • Assist with appropriate action to state survey recommendations
  • Assist in new unit openings as clinical support for concerto staff
  • Assist with grievance process when needed
  • Assist with Lab redraw follow through

CUSTOMER RELATIONS & COMMUNICATION:

  • Act as Liaison for MD and RN in facilities when needed
  • Serves as a patient advocate
  • Collaborates with RM to strengthen working relationships with RN and PCTs
  • Promotes positive customer service strategies with staff
  • Provides excellent customer service as it applies to all external and internal customers including patients, families, nursing home staff, CRS staff and physicians
  • Maintain effective communication process with Director of Operations
  • Maintain effective communication with all CRS employees including departments heads and support staff
  • Ensures strong communication between the CCO, Vice President, Directors, Regional Managers, direct patient care staff, Nursing Home administration/staff, and all support staff at the facility.
  • Routinely meets with the Vice President and Regional Manager to discuss audit results, personnel and patient care status, issues, and information.
  • Demonstrates flexibility in response to unexpected changes to work volume, acuity, staffing and scheduling.
  • Supports training and educational needs of the facility as needed.
  • Collaborate with Clinical Services Team
  • Performs additional duties as assigned.

Physical Demands of the Job

  • Must meet the necessary requirements of Ishihara's Color Blindness test as a condition of employment.
  • At times, the position provides direct patient care that regularly involves heavy lifting, frequent, prolonged periods of standing and moving of patients, and assisting with ambulation.
  • The employee may occasionally be required to move or assist with machines and equipment of up to 200 lbs. and may lift chemical and water solutions of up to 30 lbs. as high as 5 feet.

QUALIFICATIONS:

  • Registered Nurse
  • Understanding of CMS regulations and KDOQI guidelines
  • Basic Use Microsoft Office including PowerPoint, Word, Excel, and Outlook
  • Unencumbered driver’s license
  • CPR certification
