Job Profile Summary:

CAI is in search of a Case Manager for a client in the Hollidaysburg, PA. This is a direct hire opportunity.

This position is an onsite position with travel throughout Blair County.

The Case Manager will provide support to children and/or youth and adolescents experiencing difficulties including abuse, and to facilitate growth, behavioral and emotional improvements to those individuals and their families served.

Duties and Responsibilities

Provides services to individuals, families, or groups to help them attain a more satisfactory social, economic, mental, or physical adjustment to their environment.

Conducts safety assessments per Child Protective Services (CPS) laws; conducts physical, sexual, and emotional abuse investigations; conducts minimal fact interview with the victim child(ren).

Make notations in case files of each contact made, including but not limited to child(ren), parents, family members, school and, and law enforcement.

Documents interviews and uploads related information and/or documents in the Child Accounting and Profile System (CAPS).

Works with district attorneys and law enforcement officers to collaborate on cases.

Provides referrals for forensic interviews of victims and non-victim-related children in case file; attends of forensic interviews.

Assesses the safety and risk factors for every child in case for each visit/contact with child/family.

Creates a safety plan or place child(ren) when safety and risk factors are of concern for the well-being of child(ren).

Participates in MDRT, MDIT and FGDM (family group decision making) meetings with appropriate personnel including service providers, medical professionals, and law enforcement to determine further community assistance required.

Prepares for court by meeting with CYF solicitors.

Shares safety concerns of children with the courts for hearings such as dependency, shelter care, protection from abuse (PFA) orders and custody.

On-call during nontraditional hours, participates in after-hours emergency duty shift coverage.

Attends meetings, trainings, seminars, and conferences as required.

Performs other duties as assigned and/or required.

Education/Experience Required

Bachelor's degree that includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences OR any equivalent combination of experience and training that includes:12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year's experience as a social services aide or in a similar position preforming paraprofessional case management functions.

Skills

Outstanding written and verbal communication skills required.

Exceptional Customer Care, problem solving, organizational and time management skills.

Self-motivated and able to work independently, as well as in a team environment.

Ability to bring project to successful completion within appropriate timeline.

Demonstrate flexibility during times of change and ability to adjust to shifting priorities, demands and timelines.

Physical Demands

Must be able to sit for long periods during the workday, with frequent standing, walking and occasional twisting, bending, stooping, pushing, pulling, grasping, and reaching necessary to carry out job duties.

Dexterity requirements range from simple to coordinated movements of fingers/hands, feet/legs, torso as necessary to carry out job duties.

Work with varying levels of stress from very high levels to very low levels.

Moderate work, with occasional lifting/carrying of objects with weights of 40 pounds.

Must demonstrate emotional stability.
