Description

What are we looking for in a Community Health Worker?

The Community Health Worker serves as a link between Comprehensive Healthcare and the Walla Walla community to facilitate access to behavioral health services, improve the cultural competence of service delivery, and advocate for local health needs.

The Community Health Worker makes home visits to clients who have or are at high risk for developing chronic health problems and works to identify the clients’ needs and challenges, and then develops individual care plans with the client to increase engagement with treatment and provide appropriate use of services.

The Community Health Worker supports clients by developing trusting relationships, overcoming resistance to the use of behavioral health services, and empowering clients to be the driving voice in their treatment.

The ideal candidate for this role will have already established strong connections within the Walla Walla community to quickly become a trusted resource.

Strong communication skills and an understanding of social determinants of health are essential in this role. This position has a schedule of Monday-Friday 8am-5pm.

Community Health Worker duties may include :

  • developing and maintaining trusting relationships with clients in the Walla Walla community who have or at high risk for chronic health conditions
  • visiting clients in their homes to provide support and collaborating with the clients to identify needs and challenges and develop an individual care plan
  • evaluating and modifying support as necessary based on client progress and input from the client and treatment teams
  • providing motivational coaching to assist clients with developing skills to access, enroll in, and utilize resources and services related to transportation, housing, food, and other basic needs
  • assisting clients with making and getting to appointments and helping clients to overcome their resistance to utilizing services
  • completing all documentation in a timely, client-centered manner
  • supporting goal of same-day access and first appointment-to-engagement-of-treatment within four business days
  • demonstrating knowledge of CCBHC model of care for integrated behavioral health systems and national outcome measures specific to the grant

Qualifications :

  • High school diploma or GED
  • Knowledge of social determinants of health and related challenges, as gained through shared life experiences in diverse and underserved communities
  • Must complete Community Health Worker training within six months of hire
  • Must have valid driver's license, vehicle liability insurance, and a minimum of two years of driving experience
