Position Title:

Per Diem Homecare Nurse

Organizational Relationship:

The position of per diem Nurse reports to the DON and Administrator.

Position Scope:

• Evaluating client’s current functional status
• Create plan of care based on needs of client
• Perform home safety evaluation
• Conduct follow-up evaluations and supervisory visits on aides

Position Qualifications:

• Graduate of an accredited School of Nursing required
• A licensed RN in good standing, holding a current/appropriate license/registration in the state of New Jersey
• A valid driver’s license
• A valid auto insurance policy with liability coverage

Experience:

• One year of professional experience as a Registered Nurse
• Experience in home care nursing preferred

Physical Requirements:

• Able to effectively communicate with clients and co-workers in English
• Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing
• Ability to deal effectively with stress

Performance Responsibilities:

• Maintains standards of homecare and implements the policies and procedures established by the agency
• Competently assess and documents the clients biophysical, psychosocial, and educational needs
• Interprets to the client and family the implications of the client’s health status
• Demonstrates the ability to communicate effectively with other members of the health care team and staff of the agency
• Consistently adheres to universal precautions, aseptic technique, and infection control guidelines
• Consistently implements care in a manner that is safe for the client, his/her family, and self
• Consistently assumes and follows through on the responsibilities of the assignment
• Maintains confidentiality of client assessments and records
• Demonstrates sound judgment and clinical knowledge in planning and decision making
• Consistently demonstrates professionalism through appearance, performance, and communication
• Respects the rights, privacy, and property of others at all times
