Qualifications

· Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree accredited dental school

· Current/Active license in good standing in this state (or eligible to obtain)

· Dedication to integrity and achieving the highest standards of ethical patient care
