Social Worker Responsibilities:
- Conduct psychosocial assessments of residents to identify their needs, strengths, and preferences.
- Develop individualized care plans in collaboration with the interdisciplinary team to address the psychosocial needs of residents.
- Provide counseling and emotional support to residents and their families.
- Assist residents in navigating and accessing community resources and services.
- Coordinate and participate in family meetings to discuss resident care plans and address any concerns.
- Collaborate with the healthcare team to ensure holistic and integrated care for residents.
- Advocate for residents' rights and ensure they receive respectful and dignified care.
- Facilitate group activities and programs to enhance the social well-being of residents.
- Maintain accurate and up-to-date documentation of all social work interventions and assessments.
Social Worker Qualifications:
- A social worker degree required
- Previous LTC/SNF experience preferred
- Ability to work independently or as part of a group.
- Computer knowledge
