Responsibilities of a Certified Memory Care Coordinator:

• Design and implement specialized memory care programs to meet the unique needs of residents.
• Collaborate with the interdisciplinary team to create a supportive and engaging environment.
• Conduct comprehensive assessments of residents with memory-related conditions.
• Develop and update individualized care plans based on assessment findings and residents' evolving needs.
• Provide ongoing training to nursing and caregiving staff on best practices for memory care.
• Foster a culture of empathy, patience, and understanding within the memory care team.
• Communicate effectively with residents' families, providing support, information, and encouragement.
• Organize family education and support programs related to memory care.
• Monitor and evaluate the effectiveness of memory care programs and interventions.
• Implement quality improvement initiatives to enhance the overall quality of care.

Qualificationsof a Certified Memory Care Coordinator:

• Current certification in Memory Care or Dementia Care.
• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of New Jersey.
• Experience in memory care.
• Strong leadership and communication skills.
• Compassionate and patient-centered approach to care.
