Compass Health Network is currently seeking a Healthcare Home Nurse Care Manager to join our integrated care team. This position offers a $3,000 sign-on bonus, based on hours worked, as we expand our services to better serve our community.
Role OverviewAs a Healthcare Home Nurse Care Manager, you will be responsible for providing direct clinical and medical services to individuals enrolled in the Health Care Home program. Your focus will be on delivering holistic care through a person-centered approach.
Responsibilities

• Manage a caseload of up to 250 Healthcare Home patients
• Conduct health screenings and develop treatment plans for individuals with chronic diseases
• Educate clients on medical conditions, treatment options, and medications
• Collaborate with medical providers to ensure comprehensive care
• Utilize healthcare information technology tools to monitor and track patient progress
• Coordinate screenings and follow-ups as per program directives
• Collaborate with multidisciplinary teams and external agencies to provide holistic services
• Ensure culturally competent care is provided to meet the diverse needs of our clients

RequirementsEducation:

• High School/GED required
• Practical Nursing or Associate of Nursing degree required
• Bachelors degree in nursing preferred

Work Experience

• Proficiency in Microsoft Office Suite and Electronic Medical Records preferred

Licensure/Certification

• Licensed Practical Nurse (LPN) required
• Registered Nurse (RN) preferred
