In collaboration with patients, families (as defined by the patient), providers and staff across all disciplines, the person in this position is responsible for providing mental health and substance abuse interventions, completing psychosocial assessments, and addressing the social determinants of health. The person in this position serves as an integral part of the multidisciplinary healthcare team and serves as the patients’ advocate in addressing their social work and behavioral health needs. This position will serve as a fundamental team member to continue to cultivate CommUnityCare’s team-based care model initiatives. This position may require travel between CommUnityCare sites.
Essential Duties (at least 5 that are non‐negotiable duties and are absolutely pertinent to successfully completing the job without accommodations): Apply empowerment‐based prevention and intervention strategies to improve community health. Offer the “person‐in‐environment” perspective, which incorporates all factors that influence a patient’s health care experience. Assess individual needs, strengths, barriers, and readiness to change Address personal and environmental issues with the patient in order to maximize emotional, social, and physical wellbeing and the appropriate use of healthcare resources. Utilize the DSM‐5 to provide diagnostic impression for patients with mental health and/or substance use disorders. Develop and implement treatment plans. Conduct direct delivery of mental health services and behavior change interventions (which may include in therapy sessions and/or brief interventions). Conduct crisis counseling as needed. Initiate appropriate referrals for patients to outside mental health and/or substance abuse treatment as needed and/or to appropriate community‐based resources Help patients and families understand illness and treatment options, as well as consequences of various treatments or treatment refusal. Provide the expertise, consultation, and education in areas relating to psycho‐social issues affecting patient care Communicate and collaborate with all members of the care team to assess patient’s needs and provide comprehensive patient‐centered healthcare. Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete. Attend daily patient care (unit) huddles and periodic informational (clinic) huddles, meetings, patient conferences, or planning sessions. These may be related to quality assurance, patient care and other related topics within the clinic. Attend seminars and maintain all licensure and/or certification requirements for continuing education and best practices. Participate in quality strategies to evaluate compliance with evidence‐based guidelines, standards and to identify opportunities to improve patient outcomes.
Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures. Develop and maintain favorable internal relationships, partnerships with co‐workers, including clinical managers, clinical support staff, providers and business office staff. Interact respectfully and collaboratively with patients and their families, striving to develop favorable relationships with families. Seek opportunities and implement goals for own professional development. Perform other duties as assigned
Knowledge/Skills/Abilities: High level of skill at building relationships and providing excellent customer service. High degree of knowledge and competency in mental health treatment plans. High level of problem solving skill to better serve patients and staff. Strong attention to detail and accuracy. Excellent verbal and written communication skills. Demonstrated experience conducting mental health services in both single and group settings. Demonstrated experience writing and executing treatment plans. Demonstrated success in collaborating with other healthcare providers regarding treatment plans. Bilingual (Spanish/English) preferred.
MINIMUM EDUCATION: Master’s degree in Social Work (MSW)MINIMUM EXPERIENCE: 2 year’s experience in a field related to counseling individuals in crisis/trauma situations
REQUIRED CERTIFICATIONS/LICENSURE: Holds and maintains certification as: (1) Current Clinical Social Worker License (LCSW) in Texas (2) Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment.
