Rates: Fee for service

Education/Experience:

• Current NJ RN License
• Valid Driver’s License required
• One year exp. in home health required

Job Summary: Provide direct care and patient teaching related to disease process

Job Description:

• Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
• Maintain accurate, detailed reports and records including diet and physical activity
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
• Evaluate patient outcomes to determine effectiveness of treatments.
