Job Description
Our Permanent Supportive Housing programs serve individuals that have experienced chronic homelessness. The Case Managers are responsible for ensuring effective and appropriate services through participant-led goals that assist in helping maintain independence in activities of daily living within the community. This position requires excellent documentation, communication, and organizational skills. Effective time management is an essential trait as well. The Case Managers must respect each participant’s right to self-determination and operate with a non-judgmental attitude.

Qualifications
The Case Manager must possess a Bachelor’s degree plus one year of paid experience in the human service field or have at least five (5) years of case management experience with the target population. Knowledge of the targeted population and of entitlements and social services is preferred. A good driving record and adequate insurance is required.
