LOCATION: Dublin, GA (in-person); Dublin Adult Outpatient (Building 1)


Responsibilities:

  • Establish collaboration with local jails.
  • Provision of case management services, via community transition planning (CTP) for adults (18 yrs) currently in the custody of a local Sherriff’s department, presenting with a history of mental illness or symptoms of a mental illness.
  • Provision of screening to support access to necessary services, resources. (Mental health, substance abuse, SSDI entitlements, housing, HUM etc.)
  • Provision of referral and linkage to services and supports that decrease hospitalization, incarceration, homelessness.
  • Facilitate in-reach to local jails.
  • Provision of care coordination to support transition out of jail and connection to services and supports in the community including housing, social support, entitlements, transportation behavioral health etc.
  • Locate available resources and reduce barriers to accessing needed services, supports and resources.
  • Complete a need for supported housing survey.
  • Provide follow-up post release from law enforcement custody to determine access to services and successful linkage.
  • Maintain data reported monthly
  • Caseload ratio 1:40
  • The goal for direct service time is 60%. The minimal accepted direct service time is 50%. Direct service time is defined as a face-to-face billable contact.
  • All records for which you are responsible are expected to be in 100% compliance with CMHC and CARR standards. The minimal accepted compliance ratio is 90%. This will be determined by random clinical audits.

Requirements/Preferences:

  • High school diploma or equivalent.
  • Valid GA Driver's License, preferably someone 20-21 years of age to drive company vehicles.
  • Pass a drug screen and background check.
  • Basic Computer Skills
40 Hour Work Week
Monday - Friday, 8am-5pm
1 Hr. Lunch Break
