LOCATION: Looking for candidates to serve in or around Crisp County (Cordele, GA) and Chatham County (Savannah, GA) as well as Muscogee County (Columbus, GA), Thomas County (Thomasville, GA), Ben Hill County (Fitzgerald, GA) and/or Ware County (Waycross, GA).

HOPE IC3 is seeking qualified individual to serve as a Care Coordinator for their Intensive Customized Care Coordination (IC3) Program. The Care Coordinator will serve youth and families in need of customized, coordinated care in order to discover the underlying needs of the family by building upon the strengths of the family and their supports. The Care Coordinator will be responsible for engaging with assigned families and their formal and informal supports to determine all needs and strengths. Care Coordinator will use learned knowledge to create a team to produce innovative strategies to address the identified underlying needs. Care Coordinators will coordinate with all involved to ensure team moves in the same direction, everyone stays informed, and monitor needs and progress.

Care Coordinators serving at a 10 individual to 1 care coordinator ratio:

  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
  • Effective verbal and written communication skills.
  • Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to develop and deliver case presentations.
  • Ability to analyze complex information, and to define and solve problems.
  • Ability to work effectively in a team environment.
  • Ability to work in partnership with family service providers with lived experience.
  • Ability to effectively tell a family’s story by understanding and relaying their hope for future.
  • Travel over multiple counties required.

Job Requirements/Preferences:

  • Care Coordinators must possess a minimum of B.A or B.S. degree in social work, psychology or related field with a minimum of two (2) years clinical intervention experience in serving youth with SED or emerging adults with mental illness.
    • All Bachelor level and unlicensed care coordinators must be supervised at minimum by a licensed mental health professional (e.g. LCSW, LPC, LMFT).
    • Experience can be substituted for education.
  • Valid GA Driver's License, preferably someone 20-21 years of age to drive company vehicles.
  • Pass a drug screen and background check.
  • Basic Computer Skills

**Offered pay rate differs depending on qualifications such as experience and education.

40 Hour Work Week, Flexible Schedule
Monday - Friday with occasional weekends as needed.
Hours may vary depending on the served individuals' needs.
