Description

We are seeking an Admissions and Entitlement Coordinator/Director to join our team of caring professionals.

BASIC RESPONSIBILTIIES:

ADMISSIONS:

  • Responsible for overseeing all Program admissions from initial contact, screening process, program visits, admissions procedures, due process, and final placement. Complete and assure OPWDD eligibility requirements as per regulations.
  • Maintain Program databases of Residential and Day Program vacancies and coordinate review of the DDRO Emergency Needs list with the Admissions Committee.
  • Maintain and assure updates to the CHOICES software system for program participant rosters and information, ensuring that individuals are properly enrolled and terminated within that system.
  • Coordinate all participant information and changes with the Director of Billing to properly update the CRSI electronic billing records in the EVERO software program.
  • Comply with all Program visits and dinner/overnight visits; collect, review and process all data from visits and maintain continuous contact between all parties.

ENTITLEMENTS:

  • Responsible for developing proper monitoring procedures related to program participants’ benefits and entitlements to ensure compliance with funding through Medicaid and Social Security Administration.
  • Complete and maintain eligibility for individuals’ benefits and entitlements with Medicaid, Social Security and Medicare. Track expiration dates for individuals’ benefits and entitlements.
  • Develop tracking systems to submit re-determination and recertification’s of benefits as per regulatory guidelines. Ensure all individuals are eligible for entitlements before program admission.
  • Ensure resolution of all entitlement issues pertaining to Medicaid billing, exception codes, Social Security Administration and SNAP. Report unresolved issues to Chief Financial Officer.
  • Provide the Finance department with all new admissions benefits and income status regularly.
  • Visit government offices as necessary to fulfill obligations to maintain entitlements for individuals, for example: Social Security offices, OPWDD offices, etc.

Qualifications


QUALIFICATIONS

  • Bachelor’s in social work or related Health Care field. LSW or MSW preferred
  • Five years’ experience working in the field of Human Services
  • Excellent verbal, written, and technical communication skills.
  • Proficiency in excel, data management software, and experience with benefits and entitlements preferred.
  • Valid NY State Driver’s License.
