General Summary: Provide a full scope of integrated primary care services including but not limited to; diagnosis, treatment, coordination of care, preventive care and health maintenance to members of all ages.
JOB RESPONSIBILITIES:
- Obtain health history and perform physical exams.
- Diagnose and direct counseling for each member on a treatment plan.
- Arrange for referrals to specialists, labs, and imaging.
- Review and sign incoming labs, x-rays, other test and member records from other physicians.
- Participates in multidisciplinary team conferences.
- Prescribes medications.
- Assists in resolution of complaints.
- Participate in quality improvement, management, continuing education.
- Comply with all federal regulations such as, OSHA, HIPAA, and Outpatient Treatment Center licensure requirements.
- Attends regularly scheduled staff meeting.
- Performs other duties as assigned or necessary as they relate to the general nature of the position.
- Experience-Experience with Integrated Primary Care of all ages.
- Licensure-Licensed as a Physician in Arizona. Valid DEA License.
- Minimum 21 years old
- Arizona Drivers License, 39-month Motor Vehicle Report and proof of vehicle registration and liability insurance that meets insurance requirements.
- Must possess Level 1 Fingerprint Clearance Card.
- Must possess a CPR and First Aid certification
- Initial current negative TB Test result (employer provides)
