Summary/objective:The access coordinator will promote access to services and address needs related to the social determinants of health. The Access Coordinator will work in collaboration with Harmony Health Primary Care staff and leadership to ensure effective quality care with adherence to systems of care values, trauma informed principles, and agency values and mission.

Essential functions:

• Attend community functions to provide information about services to increase access including on-site scheduling.
• Oversee food pantry food delivery, food distribution, and all associated reports and paperwork.
• Arrange, and if necessary, provide transportation to Harmony Health Primary Care appointments at the Vevay location.
• Assist patients in completing applications for medical insurance including presumptive eligibility, Medicaid, Medicare, etc.
• Assist patients in reducing barriers to accessing services.
• Link patients to needed services and supports available both internally and externally.
• Attend Harmony Health team meetings and other meetings as scheduled.
• Complete all agency documentation in a timely manner.
