Summary:

Develops and monitors treatment plans with a case load of adult residential clients toward the
goal of eventual independent living. Coordinates care with the client throughout the CMHA
system of services. Supervises client activity and participation in the residential program while
ensuring client safety, comfort and satisfaction.

Essential Responsibilities and Target Outcomes:

 Provides direct care to clients by conducting individualized and group education, skill building,
recreation and household maintenance activities and/or monitoring environment with clients
for cleanliness, safety and repairs.

 Develops and reviews individualized treatment plans with clients to identify goals and progress
throughout the client’s stay by meeting with each client on the caseload at least once each
week.

 Maintains client medical records by documenting all client activity in the form of Progress Notes
and Treatment Plans in accordance with agency Policy and Procedure. Completes all additional
program required documentation, including change of shift report in compliance with CMHA
Policies and Procedures.

 Supervises and documents the self‐administration of medication to program clients as
scheduled in an accurate, professional way by adhering to program policy and procedure and
training.

 Communicates and/or advocates with other internal and external providers to ensure
consistency of and accessibility to requested/needed services. May provide transportation as
needed.

 Performs other related duties as assigned.
Principal Customers:

DMHAS Target Population of adult clients from CAC 19, Region IV who are severely and
persistently mentally ill and/or co‐occurring disordered residing in the program and their
families.

Critical Work Relationships:
All clinical and medical staff of DMHAS referring facilities/agencies. Regulatory/Accreditation
personnel. All providers of ancillary services to Residential clients. All staff of CMHA, Inc. affiliate
network as well as all CMHA, Inc. employees.

Minimum Requirements:

Education: Bachelor’s Degree in related case management field or equivalent
experience.

Credentials and
Licenses:

Relevant credentials including but not limited to, CPR, First Aid Certification.
Valid Connecticut Driver’s License.

Experience: Successful direct care experience in relevant field, preferably a minimum of
1‐3 years.
Intermediate experience with Microsoft Office products including Word and
Outlook. Experience in or ability and willingness to learn how to use an
Electronic Medical Record (EMR) system or the ability and willingness to
learn how to use one, which includes appointment scheduling,
authorizations tracking, assessments, treatment plans, progress notes and
discharge summaries.

 Ability to practice effective interpersonal skills with clients, their families
and all involved parties.

maximum/desired behavior and/or goals.

 Ability to multi‐task effectively by setting priorities appropriately.

and records in a manner consistent with professional and CMHA codes
of ethics.

 Awareness/sensitivity to all types of issues related to client diversity.
