Job Summary:
The Shared Living Services (SLS) Coordinator, you will be responsible for regular visits to the approved SLS homes to ensure that the SLS Provider is current with appropriate documentation. This includes documentation on Medication Administration, Progress Notes, Hab-Plans, Appointments, Census, and PCP goals.

The person in this role will be responsible for assessing the general home environment to ensure it meets health and safety standards. You will be responsible for participating in the Person Centered Planning (PCP) process and PCP team meetings as well as assisting providers in complying with Shared Living Services regulations. This position requires that you work closely with the Quality Assurance team and you will be the ongoing liaison between the Financial department, PCP Facilitator, and SLS Providers.

Qualifications: Associate degree in a related field, and one year of experience or a combination of education and experience with developmentally disabled individuals. Must

have excellent communication skills, both oral and written, proficient keyboard/typing skills, detail-oriented, and be able to meet deadlines. Must be able to work independently.

Supervisory skills preferred. A valid, clean Maine driver’s license required.

Job Type: Part-time

Pay: $17.25 - $19.00 per hour

Expected hours: 27 per week
