Community Horizons is assisting a fellow community member with the recruiting process for an in-home care professional. Now is the time to experience what self-directed support is all about. Less stress, no forced shifts, working one-on-one with a wonderful individual in a calm, stress-free environment.

Schedule: 5 pm-7 pm (Wednesday, Friday, Saturday, and Sunday)

Hiring multiple candidates!

Wage: $17 per hour

Responsibilities:

· Transfers utilizing a Hoyer Lift

· Meal Prep

· Personal Care including bathing and toileting.

· Housekeeping

· Laundry

· Medication Administration

· Repositioning and turning

· Range of motion exercises

· Assistance with dressing/undressing

Ideal Candidate:

· Effective, honest Communicator

· Easy-going and Patient

· Sense of humor

· Soft Spoken, not overly talkative.

· Caregiving experience

· Valid Driver’s License

· Must pass a background check.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
