Community Horizons is assisting in the recruitment process for a personal caregiver to provide top-notch care to a fellow community member in her home. The member is seeking a caring, dependable, passionate caregiver to help maintain an independent lifestyle.

Schedule: Tuesday & Thursday 9:30 am -11:30 am (with potential for weekend hours)

Wage: $15.00 per hour

Responsibilities:

· Assistance getting out of bed utilizing a slide board.

· Personal Care

· Catheter care and incontinence support

· Range of Motion Exercises

· Meal Preparation

· General Housekeeping

· Community Involvement

Ideal Candidate:

· Caregiving experience a plus

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
