Do you want to make a difference for a local community member? There is little hands on personal care needed and the hours are consistent in working around school schedules or possibly another job! Community Horizons is assisting with recruitment through the self-directed supports program, where you will work with the individual in his home to assist with completing day to day tasks. This position is hired directly by the member and not through an agency.
Hours available: 9:30am – 1:30pm, Sunday – Thursday.
Wage: $12.60/hour
Tasks may include, but are not limited to:
- Completing blood sugar checks (equipment is on site, no insulin administration required)
- Providing supervision/companion care to a young man on the Autism spectrum and engaging in socialization
- Assisting with meal preparation tasks and light cleaning
- Helping to pass meds (which are monitored/set out by another person)
- Some assistance with personal cares (e.g. toileting) may be required
Who we’re looking for:
- Someone who is reliable, dependable, and friendly
- An individual with some experience with persons on the Autism spectrum is preferred
- An empathetic, compassionate support person who wants to make a difference
- An acceptable background check
- Great communication skills and ability to take direction

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
