Overview:
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.

Expected Hours: 6 hours per week, late morning, two days per week (Flexible).

Pay: $16 per hour

You are a great fit for this position if:

• You are reliable.
• You have at least 3-6 months of experience as a caregiver.
• You are comfortable assisting with personal care tasks such as bathing.
• You can pass a Wisconsin Caregiver Background check.

Assistance needed during a typical shift:

• Bedside baths.
• Light housekeeping.
• Meal prep.
• Medication reminders.
• Assistance with dressing and undressing.
• Help manage medical supplies.
• ROM stretches.
