Community Horizons has the privilege of assisting a wonderful family in the Green Bay area to hire a self-directed caregiver to add to their care team.

The hours for this position are from 730am-11am on Sunday, Monday, Tuesday, and Saturday. Evening hours are from 730pm-930pm.

In this position, you will work alongside skilled nursing staff to meet the needs of the individual being cared for.

The pay for this position starts at $15/ hour

Job tasks:

· Assisting skilled nursing staff to care for a Tracheotomy.

· Assist skilled nursing staff in completing a bowel program.

· Empty catheter bag.

· Assist with transfers using a mechanical lift.

· Assist with personal care tasks (Showering, nail care, hair care, beard care, etc.)

· Complete range of motion stretches

· Assist with dressing and undressing.

· Assist with other care tasks as requested by the client or nursing staff.

· Complete Supportive home care tasks (Light housekeeping, meal prep, and other household chores are requested).

Additional requirements:

· Has a valid Drivers license and reliable vehicle.

· Able to pass a caregiver background check.

· 6 months- 1 year experience working as a caregiver.

· Works well in a team environment.

· Has a sense of humor!

· Experience caring for someone with a spinal cord injury.

Self-direction gives program participants the choice over who provides services and how they receive them.
