This position is within the self-direction program. Under self-directed services you work directly for the individual, which offers more work life balance and flexibility in your life. This position is hired directly by the member themselves and not through an agency.

Hours available: Monday – Friday, 4:00pm - 6:00pm and 8:30pm - 9:30pm. Pick your shift, or better yet, pick them all! This position is temporary, starting in May and ending early September.

Wage: $15.00/hour

Tasks may include, but are not limited to:

- Some assistance with personal cares, including dressing/undressing, oral hygiene, and assisting with applying and removing leg/foot braces

- Help with meal preparation, including prep and cleanup tasks

- Cues, reminders, and hands on support for daily living tasks (e.g. medication reminders and other basic household needs)

- Laundry and other light duty cleaning as needed

Who we’re looking for:

- Someone who is reliable, dependable, and friendly

- Applicants MUST be comfortable with a cat in the home (no allergies, please)

- An acceptable background check

- Great communication skills and ability to take direction

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
