Community Horizons is assisting a wonderful individual located in the Green Bay area to hire a new caregiver to join her in-home team. Entry level caregivers are welcome to apply- this would be a great place to learn new skills.

The hours for this position are:

930am-1130a- daily

2pm-330pm- Monday and Sunday weekly

930pm-11pm on Monday, Tuesday, and Wednesday weekly

The pay for this position starts at $12.00/hour

Tasks that you can expect to complete in this position:

· Use a Hoyer lift as needed.

· Complete peri care and brief change.

· Prepare meals.

· Complete light housekeeping tasks.

· Bed repositioning.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
