Schedule: Monday-Wednesday or Thursday 1:00 pm-4:00 pm

Wage: $17 per hour

Responsibilities:

· Provide life enrichment by planning and executing activities in the community.

· Building a meaningful and fulfilling relationship

· Willingness to transition to hands-on care in the future

Ideal Candidate:

· Caregiving experience

· Hoyer Lift experience

· Upbeat

· Ability to take on more hours in the future.

· Ability to pass a background check.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
