Summary:
  • Provides care according to the policies, procedures, and protocols of the organization.
  • Exhibits good written, verbal, and computer communication skills.
  • Records observation completely, accurately, legibly, and concisely.
  • Submits documentation of all patient care within established time parameters.
  • Reports any changes in the condition of patient or family situation to RN and consults immediately by telephone if the situation is urgent.
  • Follows agency medical policies, emergency orders, and nursing procedures whenever they apply to the LPN function.
  • Keeps the RN informed regarding her general care load, via case conference.
  • Prepares, administers, and records medications as agency policies permit, by WA/OR state laws regarding LPN Practice Act.
  • Assists in developing and carrying out nursing care plans.
  • Attends ongoing education to enhance home health knowledge and symptom management skills.
  • Attends staff meetings and in-services.
  • Accepts other duties as assigned.
  • Supports the Agency’s Mission and Value Performance Standards.
Qualifications:
  • Licensed as an LPN in WA and OR (preferred). 
  • Three years of home health experience preferred (equivalent clinical services experience/training considered).
  • Minimum one-year acute care setting experience.
  • Knowledgeable of Medicare and JCAHO standards of practice.
  • Basic personal computer skills to perform data entry.
  • Ability to communicate effectively and document concisely, accurately, and timely
  • All staff driving on agency business is required to have a valid driver’s license and current auto insurance.
  • CPR certification.
  • Proof of TB screening.
  • Proof of COVID19 Vaccination or bona fide medical/religious exemption.
