Community Healthlink (CHL) is looking for a Full-Time (37.5 hrs/wk) Medical Care Coordinator in Worcester.

The Massasoit Road Program is a Department of Mental Health funded residential facility serving Persons with serious mental illness who also have complex medical needs. The enhanced medical service components of the Massasoit Road Program allow Persons who have medical needs that exceed what can be managed in a supervised group living environment but do not require a skilled nursing facility to remain in the community.

The Medical Care Coordinator provides direct supervision to all ACCS CNA staff and oversees the medical components of treatment for all Persons at the Massasoit Road program.

Responsibilities

• Direct, oversee, and evaluate the work assignments of all ACCS CNA staff.
• Coordinate and oversee the completion of nursing assessments and care plans Persons residing at the program; work collaboratively with the Specialty Site Manager to ensure treatment plans thoroughly address the medical/clinical needs of all Persons residing at the program.
• Provide training and regular teaching to staff.​
• Comply with established division and program policies, procedures, and objectives.
• Attend a variety of meetings, conferences, and trainings as required or directed.
• Demonstrate use of quality improvement in daily operations.
• Comply with all health and safety regulations and requirements.
• Maintain regular, reliable, and predictable attendance.
• Maintain confidentiality. ​
• Attend pre-discharge meetings outside of the program (i.e., at Acute Hospitals, Respite Units, State Hospitals, etc.).
• Review referral packets to determine appropriateness of referrals being made and engaging with DMH/other care entities to create transition plans.​
• Work with program RN to develop care plans to address any acute needs related to a new admission or individual in transition.​
• Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, and tolerance toward all employees, patients and visitors.

Qualifications

• LPN or higher degree in Nursing required.

• 3 years' experience required with managing adults diagnosed with co-occurring medical and mental health complexities.

• Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including driving clients in a personal vehicle or a company vehicle.
