Community Health System - CA is Hiring a Social Worker 2, Home Health Near Fresno, CA

Overview:
  • All positions are located in Fresno/Clovis CA *

In this role, you will be responsible for providing psychosocial-emotional support and services to patients and families. At any given time, you will be working with 35-45 patients, meaning that you will always have the opportunity to learn and encounter a variety of patients. Individuals on this team make up half of the case management team, a group of 129 members. Each floor of our facility has 1-3 Social Worker's on hand at all times, this means you will always be encountering a variety of specialties and types of care. As a member of our team, you will serve as a point of contact in order to assist patients and family in understanding, coping and adapting to the person, emotion and bio-psychosocial issues.

Accountabilities
1. Collaborates with multi-disciplinary team in the coordination of care by evaluating, assessing and monitoring the patient care plan.
2. Monitors length of stay and associated costs per case to identify potential patient/family psychosocial issues.
3. Participates in the coordination of transfers to alternative level of care as appropriate, including acute psychiatric facilities.
4. Performs documented assessment and reassessment of the patient's status through the physician's plan of care and in accordance with CMS and California Children's Services regulations.
5. Completes clinical documentation and follow up notation in a timely manner and in accordance with policies, procedures and regulations.
6. Develops and implements individual treatment plan with patients and/or family based on mutually agreed upon goals to enhance, maintain and promote optimal psychosocial functioning throughout treatment and beyond.
7. Adheres to and adjusts plan of care as appropriate across the illness trajectory using evidenced-based, best practice psychosocial interventions.
8. Collaborates with multi-disciplinary team to develop an appropriate discharge plan based on the patient/family needs and follows up with discharges.
9. Demonstrates expertise in area assigned and provides crisis or intermittent counseling, intervention, information and referral services to patients, families and hospital staff, according to identified needs.
10. Performs initial psychosocial screening to determine if further assessment and disposition of patients presenting with mental health symptoms is required; refers to appropriate licensed clinician for writing/lifting a 5150 advisement.
11. Assesses and refers to external community resources to provide treatment to patients requiring mental health, substance abuse services, housing and/or other services.
12. Organizes and prioritizes workload based on patient and family needs, Nursing/Physician referrals, medical diagnosis, financial and community resources.
13. Facilitates Critical Incident Stress Management (CISM) intervention services as appropriate.
14. Facilitates and participates in end of life/withdrawal of life support discussions and provides referrals that may include hospice or palliative care interventions.
15. Maintains a comprehensive knowledge of the community and interagency referral network to enable an efficient transition of patients across the continuum of care.
16. Mentors peers and healthcare staff on issues related to social work practice. May provide in-services, education and training to staff/physicians as requested or appropriate.
17. Participates in interdisciplinary planning, case conferences, grant requirements, researches and/or quality improvement projects as needed.
18. Performs miscellaneous job related duties as assigned.
Qualifications:
Education
  • For Acute and all other areas:
o Master's Degree in Social Work (MSW) required
o For CBHC and 01-70702 Clinic-Special Svc:
o Master's Degree in Social Work (MSW) required or
o Master's Degree in Marriage and Family Therapy, Counseling, Psychology, Clinical Mental Health or related field with an emphasis in Marriage and Family Therapy required
Experience
  • 1 year of clinical, case management, social work or related experience required
Preferred:
  • Case management experience in healthcare/social services agency preferred
  • Experience in an inpatient or outpatient care setting preferred
Licenses and Certifications
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
Preferred:
  • ACSW - State of California Associate Clinical Social Worker preferred
  • AMFT - State of California Associate Marriage and Family Therapist preferred
  • CISM - Critical Incident Stress Management preferred
  • ACM - Accredited Case Management Certification preferred
  • OSW - Oncology Social Worker preferred
  • CHP-SW - Certified Hospice and Palliative Care Social Worker preferred
  • FCDBH-5150 - Fresno County 5150 Cert preferred
  • CCSP - State of California Children's Services Paneling preferred within 180 days of hire or transfer into position
