Community Health Resources (CHR) is Hiring a Case Manager: Strickland- Per Diem- 5725 Near Manchester, CT

JOB TITLE: Case Manager

EMPLOYMENT TYPE: Per-Diem

SCHEDULED HOURS: 1st, 2nd and 3rd shift coverage

PROGRAM/LOCATION: Strickland, Manchester, CT

ABOUT THE PROGRAM: STRICKLAND

An intensive residential program, providing 24-hour support services. Target population are individuals discharging from CVH.

DUTIES & RESPONSIBILITIES:

  • Maintains residents’ records and clerical records as assigned and in accordance with the Department of Mental Health and Addiction Services (DMHAS).
  • Provides individual case planning sessions. Facilitates, leads, or supervises recreational activities, community events, recovery meetings or other groups as assigned.
  • Assists residents in obtaining recovery support including permanent housing and employment. Transports residents, as needed.
  • Collaborates with treatment providers, significant others, ERSC and GAICM case managers, ABH and other concerned parties regarding each resident’s case plan.
  • Controls and expedites resolution of emergency situations. Assures the safety of the premises by conducting scheduled inspections of the building and grounds.
  • Provides residential oversight.
  • Provides rehab group focused on gaining life skills and issue related to addressing substance use and mental health needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.

QUALIFICATIONS:

Education: Bachelor's Degree Required

Experience: Requires previous experience relating to and working with substance abusers. Previous work history must demonstrate responsible work habits.

Licensure/Certification/Registration: Must have a valid state driver’s license. (Requires good driving record.) CPR/First-Aid Certification within six months of hire.

        COMPENSATION*:

        $17.00/hr

        • Our per diem and fee for service positions offer a flat hourly rate.
