Apply now to see what it means to us and what it’s like to work somewhere your voice is heard, your wellness is a priority, and your success matters.JOB TITLE: Care Coordinator EMPLOYMENT TYPE: Full-time, 40 hours/wk SCHEDULED HOURS: Monday-Friday, 8AM- 4:30PM PROGRAM/LOCATION: Adult Outpatient Program, Willimantic, CT PC#: 1520 ABOUT THE PROGRAM:The Adult Outpatient services provide psychiatric & substance abuse diagnostic evaluation and treatment. Services include assessment, consultation, individual, couples, group, family treatment, and medication evaluation and monitoring Duties & Responsibilities:
Coordinate intakes and services for clients. Track waiting lists for services.
Insure client activity data: attendance, absence, drug screens, med management, etc., completed and entered into the CDCS database accurately and within the contractually established time frames.
Maintain careful documentation of contacts and communication to maintain quality services.
Work as a liaison between agencies, to coordinate care for referred clients.
Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payeeship, legal services, medical services, education, and employment.
Assist with scheduling and providing transportation to and from appointments.
Maintains documentation in compliance with agency and program standards.
Participates in staff meetings, supervision
Additional duties as required.
QUALIFICATIONS: Education: Bachelor’s Degree in Behavioral Health or 3 years relevant experience required. Experience: Excellent communication skills. Excellent computer skills. Experience working with individuals in substance abuse treatment who may also have co-occurring mental health disorders is preferred. Minimum 2 years experience in the field of care coordination preferred. Licensure/Certification/Registration: Valid driver’s license is required. WHY CHR:

COMPENSATION*:$18.86 /hour min.-$28.29/hour max., dependent upon experience.
Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
