SCHEDULED HOURS: Monday-Friday, 8AM- 4:30PM

ABOUT THE PROGRAM:The Adult Outpatient services provide psychiatric & substance abuse diagnostic evaluation and treatment. Services include assessment, consultation, individual, couples, group, family treatment, and medication evaluation and monitoring
Duties & Responsibilities:
Experience: Excellent communication skills. Excellent computer skills. Experience working with individuals in substance abuse treatment who may also have co-occurring mental health disorders is preferred. Minimum 2 years experience in the field of care coordination preferred.
Licensure/Certification/Registration: Valid driver’s license is required.

ABOUT THE PROGRAM:The Adult Outpatient services provide psychiatric & substance abuse diagnostic evaluation and treatment. Services include assessment, consultation, individual, couples, group, family treatment, and medication evaluation and monitoring
Duties & Responsibilities:
- Coordinate intakes and services for clients. Track waiting lists for services.
- Insure client activity data: attendance, absence, drug screens, med management, etc., completed and entered into the CDCS database accurately and within the contractually established time frames.
- Maintain careful documentation of contacts and communication to maintain quality services.
- Work as a liaison between agencies, to coordinate care for referred clients.
- Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payeeship, legal services, medical services, education, and employment.
- Assist with scheduling and providing transportation to and from appointments.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision
- Additional duties as required.
