Community Health Centers is seeking a Licensed Clinical Social Worker/Licensed Clinical Mental Health Counselor to join the team at the Riverside and South End Health Centers located in Burlington, Vermont. Clinical Social Workers/Clinical Mental Health Workers Provide psychotherapy, primary care behavioral health brief intervention, and medical and clinical social work/mental health counseling services to patients at CHCB as part of a medical team. Conducts rapid assessment, diagnosis, provides brief counseling support and longer-term psychotherapy for co-occurring conditions, behavioral medicine interventions, crisis interventions. Conducts social work/mental health consultations in order to assess mental health and substance use disorders and makes treatment recommendations. Assists with accessing benefits, community resources, transportation and other resources as needed.
