Join Our Team as a Community Health Worker I (CHW I)

Are you passionate about making a positive impact on your community’s health? We’re seeking a dedicated and compassionate individual to join our team as a Community Health Worker I. As a CHW I, you’ll play a crucial role in connecting individuals with essential health services and promoting wellness.

Responsibilities:

  • Conduct Outreach, Engagement, and Population Health Activities:
  • Conduct outreach in public and community settings to raise awareness of Health Center services.
  • Enroll individuals in need of health care services.
  • Be prepared for occasional evening and weekend hours.
  • Maintain supplies of health center brochures and promotional materials in public display racks.
  • Provide Additional Patient Support:
  • Educate clients on Health Center services and programs.
  • Assist with registration and scheduling processes as needed.
  • Offer transportation for clients following Health Center transportation assistance protocol.
  • Safely operate the health center vehicle and maintain detailed daily activity logs.
  • Care Coordination for Health-Related Social Needs:
  • Collaborate with interdisciplinary teams to develop data-driven outreach strategies.
  • Re-engage patients in care.
  • Address Social Determinants of Health (SDOH) to improve patient outcomes.
  • Case Management:
  • Maintain accurate daily activity logs regarding client activity and destinations.

Qualifications:

  • High school diploma or equivalent.
  • Preference given to individuals with formal CHW training.
  • Proficiency in written and spoken English.
  • Bilingual skills in English/Spanish, English/Moldovan, or English/Russian are a plus.
  • Prior community outreach or medical office/interpreting experience, especially with MassHealth, Children’s Medical Security Plan, or other public health programs, is preferred.
