Community Health Care is Hiring a Community Health Worker Near Rock Island, IL

Here is your chance to improve the health and wellness of the community in which you live!?

As a Community Health Worker you will connect patients to the medical, behavioral health, and dental services they need most while providing education and outreach in a way that is culturally appropriate for a variety of communities within the QCA.

Starting rate for this position is $15/hr.

Community Health WorkerDuties Include:

  • Providing community outreach and education to coordinate healthcare and community resources for targeted populations.
  • Pandemic health navigation including COVID-19 education, outreach, contract tracing, and coordination of care and community resources
  • Initiate contact via preferred contact workflows and defined time frames set by IDPH & CHC
  • Initiate management of Resource Requests within defined time frames set by IDPH & CHC
  • Ensure services are provided in targeted communities
  • Coordinate health care and community resources for clients
  • Assist patients with establishing CHC as a Primary Care Provider
  • Utilize interpretation for any interviews of non-english speaking clients

Requirements for a Community Health Worker:

  • Excellent computer and communication skills. Attention to detail and the ability to manage multiple projects/tasks and work independently.
  • Expierence working with insurances (Medicaid, Medicare & Private Insurances)
  • Community involvement (past work or volunteer experience) and knows the resources in the community.
