Job Description:

Provides clinical care within scope of Nurse Practitioner license.

Required Qualifications:

Position requires a Master’s degree from an accredited program in nursing, a current license from the Connecticut State Board of Registration and successful completion of an accredited certified nurse practitioner program, and certification in CPR. This position requires experience as a nurse practitioner in community health center/clinic environment. Current license from the Connecticut Department of Health, Professional Board Certification, CT Controlled Substance Abuse and DEA License and certification in CPR. Bilingual preferred but not required.
Essential Job Functions:
  • Assesses patient illness by obtaining complete medical history ensuring that findings are properly recorded in the patient’s medical record. Performs physical examination, orders laboratory tests and analyzes results.
  • Diagnoses illness or health needs based on analysis and evaluation of clinical data
  • Provide direct patient care as a member of the clinical team working with
Nurses, MD’s, Medical Assistants, and Case Managers toward achieving the goals of CHWC Plan.
  • Collaborates with team members to assure quality patient care, appropriate distribution of work, and efficient patient care.
  • May concentrate in such medical specialties such as Family/Adult practice and HIV care.
  • Participates in rotation to provide services to the community in the CHWC mobile van as needed.
  • Provided patient care and treatment in collaboration with a physician in accordance with established state laws and organizational job description.
  • Refers patients for diagnostic tests and specialist.
  • Works with other staff to plan, develop and evaluate patient care and community health programs and policies.
  • Participates in health center quality improvement activities.
  • Participates in Professional Practice, Quality Improvement, Staff, and other meetings as assigned.
  • Maintains and enhances professional knowledge of theory and practice by attending conferences, educational programs and seminars.
Performance Goals:
  • Ability to problem solve and use good judgment on a case by case basis
  • Ability to effectively track client data, specialty appointments and diagnostic tests
  • Ability to effectively assess patient status
  • Aware of resources, referral options and services outside of the CHWC
Physical Effort/Environment:
Physical Demands (More than 4 hours per day)-Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, and high level of audio and visual acuity. Moderate physical activity. Requires handling of average-weight objects up to 25 pounds.
Mental Demands- Ability to cope with continually changing priorities under potentially stressful conditions.
Manual Dexterity Required-Ability to use a keyboard, computer, and telephone; use of medical equipment requiring fine motor skills.
Work Schedule:
Full-time: 40 hours per week
Communication Skills:
Requires good verbal skills to communicate with patients and staff members daily. Good listening skills to understand both patients and staff. Good penmanship and clear concise writing skills. Has a pleasant and respectful attitude when: communicating with coworkers, patients and internal/external clients; accepting direction and delegated tasks; and receiving constructive criticism. Oral and written proficiency in English.
