Description

Purpose:

Provide therapy, case management, and occasionally crisis intervention to adults with mental illnesses. This position will primarily work with adults, but may include youth clients as determined by need. The services are provided in hybrid setting (office setting and at home through telehealth platforms). Services consist of therapeutic interventions that facilitate symptom management, skill building, self-awareness, healing, identification and the use of natural supports, and use of community resources. Work Conditions:

This is a hybrid position, with services being conducted both in-person and through telehealth platforms. Ability to work at least one evening is required.

Duties and Responsibilities:

1. Provide comprehensive mental health assessment, treatment planning, therapeutic services, case management, and treatment evaluation for adults that have been diagnosed with a mental illness.
2. Interview and evaluate clients
3. Develop treatment plans collaboratively with the client by defining goals, specific outcomes, and time frames for treatment
4. Facilitate hospitalization if needed
5. Complete all necessary documentation (Mental Health Assessments, Treatment Plans, Progress Notes, etc.) in a complete and timely fashion, meeting agency standards/requirements.
6. Work collaboratively with the Clinical Supervisor and other members of the Adult Outpatient team, as well as other community service providers, in order to coordinate and integrate all aspects of care.
7. Adhere to confidentiality practices and procedures according to agency policies
8. Maintain compliance at all times with agency policies and procedures
9. Other duties as assigned by supervisor

Qualifications

Minimum Qualifications:

1. Master’s degree in social work, psychology, counseling or a related field is required.
2. One or more years of experience working with adults with mental illness required, one or more years of experience working with adults with severe mental illness preferred.
3. One or more years of experience working with youth and families preferred.
4. Knowledge and experience with Illinois Medicaid Comprehensive Assessment of Needs and Services (IM CANS) preferred.
5. Excellent interpersonal, verbal and written communication skills required.
