Community Connections is Hiring an Outpatient Behavioral Health Clinician I (LGSW, LGPC, LPC, LICSW) Near Washington, DC

POSITION SUMMARY

The Outpatient Behavioral Health Clinician I functions as part of a multidisciplinary team to serve the needs of potential and existing consumers, family, network members, and other stakeholders contacting or reporting to the agency for the purpose of receiving or navigating CC behavioral health services. OBHC Clinician I delivers direct services to support the enrollment of new consumers into the CC system of care as well as deliver services to existing consumers as a part of the outpatient services department. 

All treatment activities are performed under the supervision of an independently licensed clinician and within the scope of individuals practice and training.

This role carries a case load of adult consumers. Services are provided on-site and, in the community, as needed.

KEY RESPONSIBILITIES

  • Tracks and monitors communication from intake email, voice mail, and after hours call center to support the screening and scheduling of new admissions and the reengagement of existing consumers.
  • Pre-screens consumers by phone or in person. Screenings consist of evaluation of need for care, eligibility, motivation for treatment, and insurance coverage.
  • Provides direct clinical services to consumers including triage, psychosocial assessment, community support, treatment planning, psychotherapy/counseling, urgent or emergency service response.
  • Completes enrollment and on-going documentation into the Electronic Medical Record; verifies insurance; reviews consent for treatment; and completes clinical assessment, treatment plan, crisis plan, and orientation to the agency at-large.
  • Evaluates clinical progress and modifies method and intensity of clinical service as indicated by consumer’s progress and reaction.
  • Completes documentation and billing for all services in a timely manner and according to established standards.
  • Develop resources and relationships with external stakeholders and network members to improve referral opportunities.
  • Supports the referral process for consumers internally and externally for services as needed.
  • Teams with physicians, nurse practitioners, clinical supervisors, community support specialists, administrative support staff, and pharmacy staff to coordinate care as applicable.
  • Care coordination - communicates with external agencies and other treatment providers, especially co-located service providers, as needed.
  • Performs other administrative duties as assigned for the overall system of care, including but not limited to enrollments and discharges, assignments and transitions in care, check/debit card distribution.
  • Meets all program-specific performance and productivity expectations.
  • OBHC I may be given the opportunity to supervise a student or peer staff member at the discretion of the Supervisor.

DESIRED KNOWLEDGE/SKILLS/ABILITIES:

  • Master’s degree in social work, professional counselor or related field required.
  • Active Clinical License (LGSW, LGPC, LPC, LICSW) in the District of Columbia required.
  • Demonstrates knowledge of mental health principles and demonstrated ability to operationalize and implement these principles in mental health practice under supervision.
  • Demonstrates ability to understand clinical concepts (engagement, outreach, empathy, etc.) and interventions over time and to incorporate these into clinical practice under supervision of Clinical Manager.
  • Maintains confidentiality.
