Community Connections, Inc. is Idaho's premier agency serving adults with developmental disabilities, and we are seeking an experienced, motivated and caring Case Manager to work out of our Twin Fall's location's Supported Living Program.
Case Manager’s / QIDP’s are responsible for supporting participant success while ensuring regulatory paperwork obligations are met, including, Behavior Management program writing, guardian and team collaboration, participant satisfaction and quality assurance oversight. Our goal is to ensure we work together as a team to assist participants in achieving maximum independence.
Case Manager / QIDP’s must have experience working with individuals with developmental disabilities, including running Behavior Modification programming, and a four-year bachelor's degree in Psychology, Special Education, Sociology, Criminal Justice, Social Work, Health or another human service field; the experience and degree criteria is REQUIRED in order to apply. Case Manager / QIDP’s must have current car insurance, your own reliable transportation, and able to pass a new Criminal History Background Check. Current CPR / 1st Aid and 'Assistance with Medications' certifications are also a plus (but not required to apply).
