Community Care Physicians is Hiring a Nurse Practitioner Near Latham, NY

Description
Our Clinical Services team is growing! We are looking for a Nurse Practitioner to join our team! Position is a full time position, Monday - Friday, 8:00am - 4:30pm.
Summary: Provide clinical support to the organization as part of a centralized clinical team, based on the ever-changing needs of CCPs clinical practices. Responsibilities will include support of the task management program, annual wellness visit program, and the direct provision of clinical care either in office or via tele-medicine to cover leaves of absence as needed.
Essential Functions include, but are not limited to:
  • Participate in the central task management program.
  • Conduct telephonic Annual Wellness Visits for identified populations.
  • Provide direct patient care in the office setting as the need arises to cover provider vacancies due to planned time off and extended leaves of absence.
  • Assist in identifying and resolving gaps in care.
  • Collaborate with multi-disciplinary team to establish best practices
  • Facilitate appropriate follow up with the primary care provider
  • Review and submit encounters for visits completed by non-provider clinicians
Additional Duties:
  • Provide clinical support to the department of Clinical Services as appropriate

  • Ability to work independently.
  • Maintain a high level of confidentiality.
  • Work with all departments and sites in a collaborative manner.
  • Possess strong clinical and interpersonal skills.
  • Possess excellent organizational skills and keen attention to detail.
  • Proficiency in the use of the Microsoft Office suite, especially Excel, Outlook and Word.
  • Familiarity with electronic medical records and ambulatory care environment.
  • Strong teaching aptitude and desire to work with students.
  • A clear understanding of experiential learning.

Annual Salary Range: $100,000- $150,000

Requirements
Qualifications/Requirements:
  • License and active registration as a registered professional nurse in New York State. Certification and active registration as a nurse practitioner by the New York State Education Department (NYSED).
  • Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED, or certification as a nurse practitioner by a national certifying organization acceptable to NYSED.
  • Successful completion of pharmacology coursework acceptable to NYSED.
  • Three years of professional nursing experience in an ambulatory setting
