Community Care Hospice is currently hiring full-time Compassionate Registered Nurse Case Managers to provide patient care in and around Tangipahoa Parish and St. Helena Parish. ** The job encompasses both of these parishes, if you are not willing to cover both parishes, please do not apply.** Our hospice case managers plan, organize, and direct hospice care utilizing the nursing process. They effectively interact with patients, families, caregivers, and other interdisciplinary team members while maintaining professional standards of professional and nursing care.
-Reviews clinical referral information and responds to requests and inquiries as appropriate.
-Completes an initial comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Completes admissions and recertifications of assigned patients.
-Acts as RN case manager and assumes responsibility to coordinate patient care for assigned caseload and provides direct care to assigned patients. Also required to make PRN visits.
-Participates in on-call duties as assigned. Rotation is every sixth week. On-call compensation is provided in addition to full-time pay.
-Current unencumbered license required as well as at least three years of nursing experience, hospice experience preferred but not required.
