Community Care Home Health Services is Hiring a CDPAP Coordinator Near Smithtown, NY

The CDPAP Operations Coordinator at Community Care Home Health Services (CCHHS) provides oversight to the CDPAP department under the leadership of the Vice President of CDPAP Services to align our growth priorities and strategy for quality improvement, compliance with regulatory and accreditation requirements, human resources, and CDPAP business development.

Essential Job Functions:

  • Oversee eligibility and intake process for all CDPAP referrals from all referral sources.
  • Ensure referrals are addressed and processed responsively, consistently and efficiently.
  • Assist referrals with CDPAP eligibility as needed.
  • Collaborate with VP of CDPAP Services on operational projects, as needed.
  • Participate in meetings with Management to discuss growth and development of the department.
  • Provide weekly and monthly analysis of CDPAP department for performance growth targets, reports, and statistics in a timely manner.
  • Responsible for educating inquiries and referrals on the services we provide and accommodating them with their home care needs.
  • Supervise, lead, mentor, train, and coach to CDPAP team members, Consumers and Personal Assistants, as needed.
  • Develop and maintain relationships with community service providers and resources.
  • Understand the administration and management of office operations for home care agencies.
  • Liaison between departments (Care Specialist Team, Finance Team, Compliance Team, Business Development Team) to ensure smooth operational processes and work flows.
  • Maintain all HIPAA and confidentiality requirements for both Personal Assistants and Consumers.
  • Keep abreast of changes of the CDPAP program in the industry and legislation.
  • Perform any other duties as assigned by Executive leadership and management.

Experience / Requirements:

  • 5 years’ experience in the health care services or homecare industry.
  • 3 to 5 or more years in a management role and a history of building highly successful teams.
  • Knowledge of CRM; proficient in Microsoft Office Suite, HHA Exchange and Internet.
  • Understand the regulations governing the home care field, related to Medicare, Medicaid, and other insurance carriers.
  • Communicates effectively with employees, patients and their families, medical and community affiliates to develop positive relationships.
  • Demonstrates a commitment to maintain a high degree of patient satisfaction and strives to work as a team player.
  • Ability and experience in developing and deploying strategic plans and programs to meet/exceed established sales forecasts.
  • Strong work ethic, self-motivated, takes initiative, and proactive communication skills.
  • Strong presentation, planning and project management skills.
  • Experience in positioning, managing, and selling to all levels of staff and potential outside clientele.

Job Type: Full-time

Pay: $52,500.00 - $65,000.00 per year
