_{Duties/Responsibilities:}

• _{Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.}
• _{Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.}
• _{Conducts therapy practice within defined standards of care.}
• _{Directs the assistance of support staff to achieve patients’ functional goals.}
• _{Communicates effectively with other professional and support staff to achieve positive patient outcomes.}
• _{Practices departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.}
• _{Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.}
• _{Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.}
• _{Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.}

_EDUCATION:

• _{Doctoral program in Physical Therapy}

_EXPERIENCE:

• _{Interest in working with geriatric population}
• _{With Physical Therapy Experience is Preferred}
• _{With Skilled Nursing Experience is Preferred}
• _{At least one year of experience as a physical therapy aide is a PLUS}

_{KNOWLEDGE/SKILLS/ABILITIES:}

• _{Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.}
• _{Ability to remain calm in emergency or crisis situations.}
• _{Computer use knowledge and quick to learn new computer systems and use of electronic medical record.} 
• _{Must be able to read, write, speak, understand and communicate in the English language}
• _{Working knowledge of Medicare and other payer sources}
• _{Is able to practice unencumbered}
• _{Full knowledge of resident rights} 
• _{Exudes professionalism in presentation}
• _{Ability to multitask}

_{LICENSES/CERTIFICATIONS:}

• _{California license to practice Physical Therapy REQUIRED}
• _{BLS and CPR Certification}
• _{Is in good standing with all regulatory agencies and licensing boards}

_{PHYSICAL REQUIREMENTS:}

• _{Physical requirements of bending, squatting, twisting, stair climbing, walking, sitting, standing, lifting, and carrying items up to 35 lbs.}

• _{Ability to use equipment and techniques to safely assist patients to transfer, move in and out of bed and ambulate.}
• _{Duties require frequent reaching, stooping, bending, kneeling, and crouching.}
