OVERVIEW:

The Nurse Coordinator reports directly to the Director regarding the ongoing operations of his/her caseload. This includes the provision of case management services to eligible recipients and the administrative processing of referrals from the Illinois Department of Human Services, the Illinois Department of Public Health (IDPH) and other referral sources. Additionally, the Nurse Coordinator is responsible for interfacing with staff of various agencies providing referral, support, medical, and generic services to the clients and working in & meeting the additional requirements of the High Risk Infant Follow-up (HRIF) Program as needed.

RESPONSIBILITIES:

  • Assists with preliminary reviews of referrals, information received, and completes intake.
  • Performs Risk Reviews and case assignment for intakes, 711 & 712 Assessments.
  • Participates in the interagency relationship with IDHS and other public agencies on behalf of eligible recipients. Assists with contacts with these agencies.
  • Makes all visits to service sites and client's home as necessary and/or as required by program specifications.
  • Discusses possible service referrals with eligible recipients and families and assesses recipients' willingness to receive appropriate services.
  • Identifies service providers for assigned cases. Negotiates and arranges for the delivery of services.
  • Maintains records on his/her caseload and provides information on cases as necessary or required.
  • Upon assignment, provides intra/interagency staff training and technical assistance.
  • Promptly completes information/data in client information tracking system.
  • Enters all client appointments in the Cornerstone Scheduler.
  • Responsible for returning messages/client calls.
  • Maintains, updates individual Outlook calendar with out of office schedule and informs supervisor regarding updates to out of office, home visit schedule.
  • Completes internal and extramural reporting of client services and assessments for submission to funding sources.
  • Develops/maintains and updates comprehensive individual care plans.
  • Meets Performance Report requirements for face-to-face and home visit contacts.
  • Meets requirements for documenting client immunization education and referrals.
  • Meets Performance Report requirements for obtaining and documenting client Well Baby Visits.
  • Follows ASQ Policy (100%).
  • Meets Performance Report requirements for conducting and documenting ASQ/824 developmental assessments (100%).
  • Follows EPDS Policy (100%).
  • Meets Performance Report requirements for conducting and documenting prenatal and postpartum depression screenings (EPDS/825).
  • Enters referrals for all clients to PCP, WIC, family planning (P & G), etc.
  • Staffs all 911 cases and cases with current or previous DCFS involvement or reports to a supervisor immediately before pursuing any further action on the case.
  • Follows all FCM policies for 807 packets and 934.
  • Participates in agency and program team staff meetings.
  • Upon assignment, participates in special projects in consultation with Director/leadership.
  • Cooperates with peer staff and works as a team member to accomplish common goals.
  • Exercises diplomacy and professional demeanor when procuring or reviewing services for clients and demonstrates an understanding of the organizational structure of human service agencies.
  • May make presentations of CAU's services to groups requesting such in consultation with Director/leadership team.
  • Attends IDHS Nursing Excellence Committee meetings as assigned.
  • Participates in CAU case management staffings as well as staffings with identified service providers related to a specific client as necessary.
  • Participates in internal program evaluation and needs assessments reports, etc. including the FCM annual Quality Improvement Plan, Integrated Plan for Healthy MCH Outcomes, and any needed Plan of Correction from Audit findings.
  • May attend meetings within geographic area for the purpose of information exchange as assigned.
  • Exercises professional ability to work independently on a day-to-day basis and to develop collegial supports within the community.
  • Supervisory backup in Directors/Program Managers absence, or when delegated to do so.
  • Upon assignment may supervise Case Managers.
  • Assists Case Managers with meetings with families, agencies, etc., as needed (Consults on medical risk).
  • Upon assignment participates in case finding/referral and outreach services, including WIC and IDHS offices.
  • Remains reachable by phone until 5:00 pm during the weekdays and checks voice mail when off-site. Checks email and mailbox throughout the day when on site. May be requested to be on call during evening and weekend hours for program needs.
  • Flexibility in working hours to accommodate program needs & special events.

POSITION QUALIFICATIONS:

Bachelors’ degree in Nursing (with an active Illinois RN license) and a minimum of two years of experience with pregnant women, children 0 to 1 year of age and their families, or experience with community health, maternal and child health care. Bilingual Spanish speaking preferred.

This position requires an individual who is capable of interfacing with staff of various agencies providing referral, medical, support, and generic services to eligible recipients. A thorough knowledge of the Department of Human Services, Department of Public Health, prenatal care, infant/child development, community support and generic support is required.
